How the calculator works

Our estimates are built from three data sources: (1) manufacturer list prices published by Novo Nordisk and Eli Lilly, (2) NovoCare and LillyDirect cash-pay subscription programs, and (3) CMS Medicare Bridge pricing scheduled for July 2026. For commercial insurance, we assume you qualify for the manufacturer savings card that caps copays at $25/month. For Medicaid, we use a representative typical copay — your actual cost varies by state.

How accurate is the GLP-1 Cost Calculator?

Estimates are based on April 2026 manufacturer list prices, published NovoCare and LillyDirect cash-pay programs, Medicare Bridge pricing (effective July 2026), and typical Medicaid copays. Your actual cost may vary by pharmacy, plan formulary, and state.

Why does Ozempic cost $998 without insurance?

Ozempic's list price set by Novo Nordisk is approximately $998 for a one-month supply of pre-filled pens. Without insurance, most patients pay close to this amount unless they qualify for a manufacturer savings card or patient assistance program.

What is the Medicare Bridge program?

Starting July 1, 2026, Medicare Part D enrollees with a qualifying weight-related diagnosis (heart disease, kidney disease, or obesity with comorbidity) will pay a flat $50/month copay for GLP-1 medications through the Medicare Bridge program negotiated by CMS.

Is compounded semaglutide cheaper than Ozempic?

Yes. Compounded semaglutide typically costs $199–299 per month regardless of insurance, compared to $998/month for brand-name Ozempic without insurance. Compounded versions are prepared by licensed pharmacies and are legal when there's an FDA-recognized shortage.

Does the calculator include shipping and consultation fees?

Can I use a manufacturer savings card with no insurance?

No. Manufacturer savings cards for Ozempic, Wegovy, Mounjaro, and Zepbound require commercial insurance. Cash-pay patients should instead look at NovoCare ($499/mo Wegovy, $349/mo Ozempic) or LillyDirect ($349–499/mo Zepbound single-dose vials).
